Instructions

The campus clearance process will initiate upon submission of this form. All fields, except Employee ID, are required.

All information completed on this form will be emailed automatically to the Faculty Affairs office. The contact information section will be emailed to the following departments to initiate the clearance process:

  • Purchasing (for ProCards)
  • Information Technology
  • CMS (PeopleSoft account and access)
  • Library
  • Financial Services
  • Enterprises (for wolfbucks account)
  • Seawolf Service Center

The Campus Clearance Checklist will display after the submission of this online form. Please print out and complete the checklist, then submit it to Faculty Affairs on your last day of work.
